Preoperative Findings:
- Nasal Tip Ptosis: The nasal tip angled downward, giving the nose a “droopy” appearance and affecting the overall nasal-facial aesthetic balance.
- Wide Nasal Tip: The broad tip detracted from a refined nasal shape, diminishing definition in frontal and oblique views.
- Dorsal Hump Deformity: A hump along the nasal bridge disrupted the patient’s profile line and contributed to facial disharmony.
- Low Radix: The nasal starting point (radix) was low, causing a less defined and harmonious transition between the forehead and nasal bridge.
- Functional Breathing Issues: The patient also reported difficulties with nasal breathing, potentially related to internal structural imbalances.
Surgical Objectives and Plan:
- Raise the Nasal Tip: Elevate and support the tip using suturing techniques and possible cartilage grafting to eliminate the droopy appearance.
- Refine the Tip Width: Recontour and reshape the tip cartilages to create a narrower, more defined tip while maintaining structural integrity.
- Correct the Dorsal Hump: Reduce and smooth the hump to produce a straighter, more aesthetically pleasing profile.
- Augment the Radix: Slightly raising the radix (using a cartilage graft or other appropriate technique) to ensure a balanced transition from the forehead to the nasal bridge.
- Improve Airflow: Address any internal nasal valve collapse or septal deviation to enhance the patient’s breathing function.
